SMS DANCE PROGRAM FORM 2018-2019

Please make sure you have this form with you and have answered all the questions on a separate paper.  This form also needs to be fill in by the students going into Gr 8 Dance.

Each student needs to attend BOTH audition dates.  May 22 and 24.
